Who is eligible to join?
All hospital employees, guests, and visitors of the hospital may join. However, employees of Lori's Hospital Gift Shops and their families and volunteers of the hospital may not participate.
How do I join?
Simply visit your Hospital Gift Shop and complete an enrollment form. You will be issued your personal key tag instantly and you can begin to earn your Rewards Point immediately.
How do I earn Rewards Points?
Every time you make a purchase at your Hospital Gift Shops, present your personal key tag to the Sales Associate at the register. It's that simple! You will automatically receive one Reward Point for every dollar spent on eligible merchandise (see below). When you accumulate two hundred (200) Rewards Points, you will receive, by mail, a ten dollar ($10.00) Rewards Coupon, redeemable for merchandise at any of our nationwide locations.
What purchases qualify?
All products sold in the gift shop except: transit tokens/passes, parking passes, movie tickets, theme park tickets, stamps, tobacco products, purchase of a gift certificate(s), magazines, newspapers, paperback novels, 'Sale' and/or 'Clearance' items. Sales tax is excluded.
Do I have to present my key tag?
Your personal key tag must be presented each time. Store personnel can not retain your tag or personal tag information on file or allow other customers to utilize you key tag. Rewards Points are only awarded at the time of purchase and can not be added or adjusted for previous sales.
When will I receive my Rewards Coupon?
Once a month, your account is reviewed for accumulated Rewards Points. If your balance exceeds the minimum requirement of 200 points, you will be sent, by mail, a Rewards Coupon. If your balance is less than 200 points, the balance will be rolled over to the following month. Rewards You should receive your coupon around the 25th of the following month.
How do I redeem my Rewards Coupon?
Simply sign your coupon and present it to our Sales Associate on your next visit. The amount of the coupon will be deducted from your current purchase. The Rewards Coupon is valid for merchandise only and any remaining balances can not be refunded.
Is there an expiration date?
Yes.
What if I lose my key tag?
You may apply for a new key tag. However, for security reasons, we can not transfer balances from your lost key tag to your new key tag.
What if I lose my Rewards Coupon?
Sorry, we can not be responsible for lost, stolen, or damaged Rewards Coupons.
What if I move?
You must go to the gift shop and request a change of address. For your security we will not change any address via email or telephone conversation. We can not be responsible for undeliverable addresses or expired mail forwarding services.
Can my friends use my key tag?
No, the Rewards Program is designed for the sole use by the person named on the key tag application. Unauthorized purchases by other parties may result in the suspension of benefits and forfeiture of all accumulated Rewards Points associated with this program.
